I'm guessing that current and higher numbering levels have been mixed up in the "Enter formatting for this number" box for the third numbering level.
Place the insertion point in the first top-level paragraph of your list (for example, if this is heading numbering, place the insertion point in the first Heading 1 paragraph of the document). On the Home tab, click Multilevel List, and then click Define New Multilevel List. You are presented with a dialog box similar to the Customize Outline Numbered List dialog of previous versions (and you may still find the article at LINK useful).
Click level 3 in the preview picture and clear the current contents from the "Enter formatting for this number" box (by selecting it and pressing Delete), and then reinsert the numbering from levels 1 and 2 (via "Include level number from") as well as the current level number (via "Number style for this level"). When you are done, click OK.
Does that fix the problem?
